1. Permission to Participate
I, the parent or legal guardian named below, give my permission for each of the child(ren) named on this registration to attend and participate in the Parents Night Out / Buddy Night event at Personal Mastery Martial Arts ("PMMA"). I understand the event includes martial-arts-themed games, group play, themed activities (such as Nerf Wars, LEGO building, or other monthly themes), pizza and snacks, and a movie. Children will be supervised by certified PMMA instructors at all times.
2. Assumption of Risk
I understand that participation in physical activity and group play involves inherent risks, including but not limited to bumps, bruises, sprains, falls, and minor injuries that may occur during games and activities. I voluntarily assume all such risks on behalf of my child(ren). I further understand that themed activities (e.g. foam-dart play, building activities, group games) may involve incidental physical contact with other children or equipment.
3. Medical Acknowledgement & Authorization
I represent that each child named on this registration is in good general health and able to participate in supervised physical activity. I have disclosed all known allergies and dietary restrictions in the registration form. In the event of injury or medical emergency where I cannot be reached immediately, I authorize PMMA staff to administer reasonable first aid, summon emergency medical services, and authorize emergency medical treatment for my child(ren) on my behalf. I accept full financial responsibility for any resulting medical care or transport. I release PMMA from any duty to provide medical treatment beyond reasonable first aid.
4. Allergy & Food Acknowledgement
I understand that PMMA serves pizza, snacks, and beverages at this event. I acknowledge that the food served is prepared in environments that may contain common allergens (wheat, dairy, soy, nuts, eggs). I have disclosed any known food allergies or dietary restrictions on the registration form. I accept responsibility for any risk arising from food consumption and will provide my child with alternative food if needed.
5. Pickup Authorization
I authorize the pickup contact named on this registration to take my child(ren) home from the event. PMMA staff will not release a child to anyone other than the parent/guardian or the named pickup contact without my prior written or verbal authorization. I understand that pickup is at 8:00 PM (or the time stated for the specific event) and that late pickup may result in a fee. If my pickup contact is the same as the parent/guardian, I have indicated that on the form.
6. Release & Waiver of Liability
To the fullest extent permitted by Utah law, I hereby release, waive, and forever discharge Personal Mastery Martial Arts, its owners, instructors, employees, agents, volunteers, and the owner of the premises (collectively the "Released Parties") from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) sustained by my child(ren) while participating in this event, whether caused by the negligence of the Released Parties or otherwise. I understand this is a complete release of liability.
7. Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, losses, damages, costs, or expenses (including reasonable attorneys' fees) brought by me, my child(ren), my spouse, my heirs, or any third party arising out of my child(ren)'s participation in the event, except to the extent caused by the gross negligence or willful misconduct of the Released Parties.
8. Photo & Video Release (Optional)
PMMA may take photographs or video during the event for marketing, social media, and promotional use. Granting permission is OPTIONAL — your child(ren) may attend without consenting. If you check the optional box below, you grant PMMA a perpetual, royalty-free license to use your child(ren)'s likeness in such materials. If you do not check the box, PMMA will make reasonable efforts to keep your child(ren)'s likeness out of published materials.
9. Behavior & Removal Policy
I understand that PMMA reserves the right to contact me to pick up my child(ren) early if their behavior endangers themselves, other children, staff, or property. No refunds will be issued for early removal due to behavioral issues. I will instruct my child(ren) to follow staff directions and treat other children with respect.
10. Parental Authority & Electronic Signature
I represent that I am the parent or legal guardian of each child listed on this registration and that I have full legal authority to sign this waiver on their behalf. My typed name below serves as my electronic signature under the Uniform Electronic Transactions Act (Utah Code 46-4). I have read this waiver in full, I am at least 18 years old, and I am signing freely and voluntarily.
I have read and understood this Parent / Guardian Liability Waiver & Permission Form in full. I am the parent or legal guardian of each child named on this registration. I acknowledge that I am signing it freely and voluntarily, that no oral representations have been made to me, and that this is a binding legal document covering every child I have registered for this event.
